Job Description:

Health Crest Homecare, LLC is now hiring an experienced Certified Home Health Aide to join our team of passionate and service-oriented Healthcare Professionals.

Duties and Responsibilities:

  1. Provides personal care and homemaking services essential to the patient’s health care and comfort at home, under the direction, instruction, and supervision of an RN, including shopping, errands, laundry, meal planning and preparation, serving meals, and assisting the patient with ADLs.
  2. Tasks to be performed by a CHHA must be assigned by and performed under the supervision of an RN who will be responsible for the patient care provided by the CHHA.
  3. Follow the plan of care to provide safe, competent care to the client.
  4. Helps the patient to maintain good personal hygiene and assists in maintaining a healthy and safe environment.
  5. Plans and prepares nutritious meals, grocery shops, when instructed to do so by the nurse.
  6. Assists the patient with ambulation, as ordered by the physician and approved and supervised by the nurse.
  7. Assists the therapy personnel, as needed, with rehabilitative processes.
  8. Encourages the patient to become as independent as possible, according to the nursing care plan.
  9. Attempts to promote patients’ mental alertness through involvement in activities of interest.
  10. Gives simple emotional and psychological support to the patient and other members of the household and establishes a relationship with the patient and family, which transmits trust and confidentiality.
  11. Reports any change in the patient’s mental or physical condition in the home situation to the staff nurse or to the Aide supervisor.
  12. Performs routine housekeeping tasks, as related to a safe and comfortable environment for the patient. As instructed by the nurse.

Qualifications:

  • High School Diploma or Equivalent
  • Must have completed a homemaker home health aide training program approved by the NJ Board of Nursing and shall be certified by the Board and provide evidence of certification.
  • Must have a valid physical examination within the last year, TB testing or negative chest x-ray, and must have proof of MMR Vaccine.
  • Must be vaccinated with the COVID-19 vaccine (2 Doses).
  • Must comprehend the basics of personal care, housekeeping, and meal preparation.
  • Must understand and respect clients, including ethics and confidentiality of care.
  • Must have current CPR certification. Online certification is not accepted
  • Must pass a criminal background check.
  • Must have available reliable transportation, a valid driver’s license, and proof of insurance.
  • Have two (2) Verifiable professional references.
